Joe N. is a 48 year old white male who was first seen more than 2 years ago. Weighing 295 pounds, he was short of breathe just walking in the mall. His blood tests showed he was borderline diabetic with an elevated fasting insulin of almost 40 (normal less than 10). His wife knew he was in trouble, so she brought him in to see me.Joe was found to be relatively hypogonadal: that is his free testosterone level was very low. He had developed gynecomastia (enlarged breasts) and an enlarged abdominal girth. His blood also showed high levels of estrogen and the sex hormone binding globulin that prevented the good action of his testosterone.
Joe understood his hormones were 'not right.' He wanted to exercise but was always too tired. Our first treatment was a gentle walking program and a change to the Atkin type diet: low, low carbohydrates, 5-6 small meals, more protein and omega-3 fats.
Joe dilligently followed the diet even though his fatigue continued.
Next, we treated Joe with FDA approved testosterone pellets and testosterone injections. The logic was that by adding testosterone, as a subcutaneous product, there would be stable levels of testosterone. The biochemical basis was also that only injections and pellets lower sex hormone binding globulin(SHBG). The testosterone patches do not. By lowering the SHBG I hoped to lower his estrogen levels as well.
The results of seven months of testosterone in the high normal range is seen below. Joe lost 85 pounds before stabilizing at 225 pounds (70 pound weight loss). He not only looks and feels better, his hormone levels of fasting insulin, SHBG and estradiol are greatly improved. He realizes that these tests may never be normal but Joe feels as if he has a 'new lease on life.'
Joe is able to exercise on a treadmill, running, for 90 minutes! He feels he is 10 years younger! He has an excellent attitude and has encouraged his wife and daughter to exercise with him. And he can tell when he needs to replenish his testosterone by the decrease in his sexual arousal.
Testosterone is not a 'weight loss' agent. Many other patients never change their diet nor lose weight. But for Joe N. and those who use the renewed energy of appropriate hormonal replacement, feeling better leads to eating better and exercising.
